Cap-Gap Extension Request Form

The form below is to be used in order to request a new form I-20 from the ISSO which reflects your ‘cap-gap’ or the period of time between the end of your OPT and the start date of your approved or receipted H1B employment status.  You may only proceed with this form under one of the following conditions:

  1. Your H-1B petition has been filed and receipted by USCIS
  2. Your H-1B petition has been filed by your employer but not receipted and your OPT expires before June 1

In order to proceed, you must first have in your possession the following documents and be prepared to scan and upload them together with this request form:

  • H1B approval notice or receipt notice (form I-797)
  • Clear, legible copy of your most recent EAD

Once you click submit please allow 5-10 business days for review of your application and I-20 processing.  In most cases the I-20 will be automatically mailed to the address you indicate below.  Sometimes there are delays if the H1B information has not yet been updated in SEVIS.  If this is your case, you will receive an email notification with further information.  Please watch your inbox for updates. Otherwise we ask that you please allow the full 5-10 days for review and processing of your application. Thank you!

Employer Information

The Employer Identification Number (EIN) can be found on your W-2 form or obtained from your employer. It is 9 characters long with a dash after the first two characters, in the format: 00-0000000
You must provide the full name of the "representative" who will pick up your I-20. The name you provide must match the name on the representative's picture ID.
